Online Physical and Athletic Forms
Celina Schools Athletic Department has partnered with FinalForms to process all athletic forms electronically. This program allows information to stay with the athlete throughout their school career. Forms will be re-signed yearly and are able to be updated as needed. This will involve all student-athletes in grades 7-12.
At this time, forms may be reviewed and signed by both parent/student, and sports selected for the upcoming 2023-24 school year.
ALL student-athletes and their parent/guardian need to be registered and forms electronically signed prior to participation in a sport at Celina Schools. Physical forms will be printed off directly from FinalForms, signed by the doctor at your physical, and then turned in to the Athletic Department PRIOR to practice or tryouts.
ALL students wishing to participate in athletics must have a yearly current OHSAA Physical Form on file in the Athletic Department.
