The Celina Athletic Department has partnered with FinalForms to offer an online program which will enable parents and athletes to complete and file athletic forms electronically. With the exception of the physical form, all Celina athletic participation forms will now be completed online through FinalForms. Data is saved from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. Your forms may be reviewed at any time throughout the year to verify accuracy and to update as needed.
Physical forms will be printed off directly from FinalForms, taken to your doctor at the time of your physical, and then returned to the Athletic Department PRIOR to practice or tryouts.
**ATHLETES MAY NOT PARTICIPATE UNTIL ALL FORMS ARE COMPLETE**
FinalForms will open 5/01/23 for all returning students. At this time, parents and students will be able to log-in to their accounts, review and sign all forms plus select sports for the upcoming 2023-24 school year.
