Students enrolled in College Credit Plus or Home School programs MUST have their Eligibility Verification Form completed and submitted to the Athletic Department every 9 weeks to determine eligibility to participate in sports. Due dates for each 9 week period are noted on each form. FAILURE TO SUBMIT ELIGIBILITY FORM WILL MAKE ATHLETE INELIGIBLE.
